Admissions Paperwork Required

Documents required to complete assessment:
  1. Photo ID (Driver's License, Non-Driver's ID or Passport

  2. Birth Certificate

  3. Social Security Card

  4. Copy of Insurance Card(s)

For Residential Services ONLY
  1. Proof of application for Temporary Assistance with caseworker contact information

  2. LOCADTR

  3. Referral Letter

  4. Phone Screening

  5. Psychosocial/Comprehensive Psychiatric Evaluation

  6. Medical History & Physical

  7. Drug Screen

  8. Intradermal PPD

  9. List of Current Medications

  10. Mandate Letter (if applicable)
